From 1st June 2019, without exception, we are no longer able to register new out of area patients on our practice list.
Providing you reside within our Practice area (please see the map below) you can register with us. It’s easy! Simply call into either of the Surgeries and complete a Registration Form for each new patient, register online or on our online booking system.
On completion of the Form, you and your family will be registered on our computer system and will be eligible for medical assistance straight away.
Your medical records will be automatically transferred by the Health Authority from your previous Practice and you will be asked to make a New Patient Check. This is a basic 20 minute appointment with one of our Practice Nurses to gather medical history etc and to welcome you to our Practice.
Please note that the map below is a window into another website which we have no control over. You may find, particularly on mobile devices, that you may need to drag the map left to enter details and click.
Please note that cookies have to be agreed to, to see the map below
If you wish to register, click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
WHEN YOU REGISTER, PLEASE ENSURE YOU FILL OUT THE MEDICAL QUESTIONNAIRE.
This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
It also allows us to have more information on you- for example your mobile telephone number, which gives us more options to contact you later on.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
We provide a Temporary Patient service for anyone staying within our Practice area. This could be whilst on holiday, staying with relatives etc. Just call into the Surgery, fill in one of our Temporary Resident Forms and we will assist you.
